The fee for the 3 week, 3 hour per day program is $4950.00.  A deposit of $1000.00 must be made to reserve a session.

Cancellations:

If the session is cancelled for medical reasons, the deposit may be used toward the scheduling of a future session.

If the session is cancelled for any other reason by the caregiver, we will not be able to refund the deposit or use it toward a future session.

Scheduling Process:

Call our office to request a scheduling packet.

The following are also required:

• Patient information form (available in packet).
• Report regarding hip x-rays that were taken within the previous 6 months  and  

              must contain an A/P view of your child's hips to rule out hip subluxation or

              dislocation. (only required for those children using the TheraSuit™)

• If your child has scoliosis, a spinal x-ray report is also required.
• Doctor's orders and current physical therapy evaluations and notes should be

              sent with the patient information form.
